Effects of Tear Film Lipid Layer on Quality of Vision

Abstract
Purpose : This study was to investigate the effect of the tear film lipid layer on the quality of visual acuity in normal eyes.

Methods : The subjects were 30 (60 eyes) who had never been diagnosed with any ophthalmic disease, including dry eyes, and had corrected visual acuity of 20/20 or higher. The refractive errors, tear film lipid layer thickness, and contrast sensitivity was examined, and the lipid layer thickness of the tear film was quantitatively measured and divided into three groups(15~30, 30~80, £¾80 nm). Contrast sensitivity was tested for spatial frequencies of 1.5, 3, 12, and 18 cycles per degree (cpd) under photopic condition.

Results : In the measurement of tear film lipid layer thickness, 22 patients (44 eyes) belonged to groups 1 and 2 thinner than 80nm, and only 3 groups thicker than 80 nm showed normal contrast sensitivity. As the thickness of the tear film lipid layer increased, it was confirmed that the contrast sensitivity was improved in all spatial frequencies. The thicker the tear film lipid layer thickness, the higher the degree of refractive error.

Conclusion : The effect of instability of the tear film lipid layer thickness on the decrease in contrast sensitivity in corrected visual acuity of 20/20 or higher was confirmed. Therefore, it is thought that a more careful effort to maintain a stable tear film during conventional visual acuity tests and parallel tests of the lipid layer thickness of tear film and contrast sensitivity will help to provide better visual acuity.
